If you report a new claim or reopen a claim after May 10, 2013 with Lumbermens Mutual Casualty Company, in Liquidation, American Manufacturers Mutual Insurance Company, in Liquidation or American Motorists Insurance Company, in Liquidation, a proof of claim form will be sent to your attention. Please follow the instructions in completing the proof of claim form. The proof of claim form must be completed and filed with the Office of the Special Deputy Receiver ("OSD") on or before November 10, 2014 at 4:30 p.m. C.S.T. in accordance with the terms of the Order entered on August 1, 2013. Copies of the proof of claim form are also available on the OSD website at www.osdchi.com.
