Remember to provide prescriptions to your new healthcare provider

Individuals who participated in the CIGNA/Caremark health plan offered by Kemper in 2004 will need to submit outstanding claims prior to December 31, 2005. Previous Plan language allowed claims to be submitted for up to 24 months from the date of service. However, since Kemper no longer offers a health plan through CIGNA/Caremark all claims incurred in 2004 must be submitted by December 31, 2005 to be considered for payment.

Claims should be handled as they have been in the past, with healthcare claims sent directly to CIGNA and prescription drug claims sent directly to Caremark. Click here for a complete copy of the medical plan amendment .
