Due to the change in claim administrators for the
medical plan, any medical or prescription drug expenses incurred
in 2004 should be submitted to CIGNA/Caremark by December 31, 2005.
Any claims not received prior to December 31, 2005 will not be considered
for reimbursement.
Claim forms are available on the company intranet
under “Employee Info” or by contacting CIGNA or Caremark
directly.
This notice constitutes an amendment to the
Summary Plan Description for the medical plan in effect prior to
January 1, 2005.
